Apple cider vinegar for glucose and weight loss: hit or myth?

For several years now, apple cider vinegar has been a regular fixture among the most popular ways to flatten the glucose curve, curb appetite and speed up weight loss. On social media you can find advice to drink it before every meal, start the day with it, or use it as a natural substitute for medication that affects blood glucose.

Can one or two tablespoons of vinegar really improve your metabolism in a noticeable way?

Research suggests that vinegar may slightly blunt the post-meal rise in glucose, especially after a meal high in easily digestible carbohydrates. The evidence on weight loss, however, is far weaker and less consistent. So apple cider vinegar is not a complete myth, but its importance is considerably smaller than the internet would have you believe.

What actually is apple cider vinegar?

Apple cider vinegar is made through a two-stage fermentation of apple juice. First, yeasts convert the naturally occurring sugars into alcohol, and then acetic acid bacteria turn the alcohol into acetic acid.

It is the acetic acid, rather than any special property of apples, that is responsible for most of the vinegar's potential metabolic effects. Wine vinegar and other types of culinary vinegar can therefore have a similar effect. A typical vinegar contains around 4–6% acetic acid.

Unfiltered apple cider vinegar may contain the so-called "mother", a cluster of bacteria and fermentation products. There is no good evidence, however, that the presence of the "mother" meaningfully increases the vinegar's effect on blood glucose or the rate of weight loss.

How might vinegar affect glucose?

The potential action of vinegar does not come from a single mechanism. Acetic acid may influence the pace of digestion, gastric emptying, the activity of carbohydrate-digesting enzymes, and how tissues use glucose.

Slower gastric emptying

Vinegar may slow the passage of food from the stomach into the small intestine. As a result, carbohydrates are digested and absorbed more slowly, and glucose can appear in the blood more gradually.

This mechanism may blunt a sharp post-meal rise in blood glucose, but it also has a downside. In people with gastroparesis, that is delayed gastric emptying, vinegar can worsen feelings of fullness, nausea and digestive problems.

Effect on starch digestion

Acetic acid may partly reduce the activity of enzymes involved in breaking down starch. This means glucose from bread, rice, pasta or potatoes may be released a little more slowly.

The effect is probably far less pronounced with a meal that contains little carbohydrate, or after consuming plain sugar in a drink.

Possible improvement in insulin sensitivity

Some small studies suggest that acetic acid may improve glucose uptake by the muscles and temporarily increase tissue sensitivity to insulin. The mechanism, however, is neither well understood nor strong enough to treat vinegar like an anti-diabetic drug.

Does vinegar really lower glucose after a meal?

The most convincing data concern precisely post-meal glucose.

A meta-analysis of clinical trials found that consuming vinegar with a meal may reduce the post-meal glucose and insulin response. The benefit was particularly visible after meals containing carbohydrates. At the same time, individual studies differed in the type of vinegar, the dose, the composition of the meal and the characteristics of the participants, so it is not possible to point to a single effect that will occur in every person.

In practice this means that adding vinegar to a meal may, in some people, slightly reduce the height of the post-meal glucose peak. It does not mean that vinegar:

  • "cancels out" the sugar in a meal,
  • prevents the absorption of calories,
  • lets you eat unlimited amounts of sweets without consequences,
  • cures insulin resistance,
  • replaces medication used in diabetes.

The effect of vinegar is an addition to the overall pattern of eating, not a way to neutralize a poorly composed meal.

Does apple cider vinegar lower fasting glucose and HbA1c?

Some studies in people with type 2 diabetes point to a possible small reduction in fasting glucose and glycated haemoglobin HbA1c. A more recent meta-analysis of controlled trials also showed an improvement in some glycaemic parameters, but the authors noted the heterogeneity of the available data. The studies involved relatively small groups, were short, and used different doses and vinegar preparations.

It is also important to distinguish statistical significance from clinical relevance. A small improvement in the average result of a study does not have to translate into a clear health change in every person.

Vinegar can be part of the diet of someone with disordered carbohydrate metabolism, but it should not replace:

  • the treatment prescribed by a doctor,
  • an adequate intake of energy and fibre,
  • regular physical activity,
  • sleep,
  • reducing excess body weight,
  • monitoring of blood glucose and HbA1c.

People taking insulin or medications that can cause hypoglycaemia should not experiment with large doses of vinegar without consulting a specialist.

Apple cider vinegar and insulin resistance

Insulin resistance is not a condition you can "cure" with a single product. It is a disorder influenced, among other things, by the amount of visceral fat, physical activity, genetic predisposition, sleep, diet, medications and coexisting conditions. Vinegar may temporarily reduce blood glucose after a specific meal, but this does not automatically mean a lasting improvement in whole-body insulin sensitivity.

The most effective measures for insulin resistance remain unspectacular but well documented:

  • reducing excess body weight, if indicated,
  • regular strength and aerobic training,
  • an adequate amount of fibre,
  • limiting excess ultra-processed foods,
  • matching energy intake to needs,
  • an adequate amount and quality of sleep.

Vinegar can be an addition to these measures, but not a substitute for them.

Does apple cider vinegar help with weight loss?

The evidence on weight loss is far less certain than the data on post-meal glucose.

Some small studies observed a slight reduction in body weight, BMI or waist circumference. Possible mechanisms include increased satiety, slower gastric emptying, or poorer meal tolerance leading to a spontaneous reduction in how much is eaten.

These results should, however, be interpreted with great caution. The studies often:

  • involved a small number of participants,
  • lasted only a few weeks or a couple of months,
  • differed in the dose and form in which the vinegar was given,
  • did not always tightly control diet and activity,
  • concerned selected groups, for example people with type 2 diabetes,
  • showed considerable heterogeneity of results.

A 2025 meta-analysis suggested the possibility of a small, short-term improvement in body weight and some anthropometric measures in people with excess weight or metabolic disorders. The authors themselves, however, stressed the limited number of studies, the short observation period and the need for larger, better-designed clinical trials.

For this reason, apple cider vinegar cannot be regarded as an effective weight-loss agent comparable to a properly planned dietary intervention, physical activity, behavioural therapy for eating patterns or pharmacological treatment of obesity.

The important story of a retracted study

In 2024, a study attracted a lot of attention by suggesting that small daily doses of apple cider vinegar could lead to significant weight loss over 12 weeks. The results were widely cited in the media and used as an argument confirming the "slimming" effect of vinegar.

In 2025, however, the publication was retracted.

BMJ Group reported that during a review it had identified problems with the statistical analysis, implausible values, the quality of the source data, the way the methods were reported, and the lack of prior registration of the study. Independent statisticians were unable to reproduce the presented results. The publisher explicitly stated that the data from this work should not be used any further.

This is an important example showing that a single spectacular study should not outweigh the body of evidence as a whole, especially when its results diverge significantly from earlier work.

How to use vinegar as safely as possible?

If a person tolerates vinegar well and wants to include it in their diet, it is best used as an ingredient in a meal.

Studies most often used around 10–30 ml of vinegar per day, roughly 2–6 teaspoons. This does not mean, however, that a larger dose will produce a better effect.

The safest rules are:

  • do not drink undiluted vinegar,
  • do not take concentrated "shots",
  • do not hold vinegar in your mouth for long,
  • do not use it repeatedly throughout the day,
  • dilute a small amount in a large volume of water or add it to food,
  • rinse your mouth with water after consuming it,
  • do not brush your teeth immediately after contact with an acidic drink,
  • stop using it if pain, heartburn, nausea or throat irritation appear.

Vinegar is a food product, but its acidity means that using it incorrectly is not harmless.

Possible side effects

Enamel erosion

Regular contact with acidic drinks can lead to a gradual loss of enamel. The risk increases when the vinegar is undiluted, sipped in small amounts over a long time, or held in the mouth.

Damaged enamel does not regenerate on its own. Tooth sensitivity, pain and greater susceptibility to damage may appear.

Throat and oesophageal irritation

Undiluted vinegar can cause burning, pain and damage to the mucous membrane. Cases of oesophageal injury have been described after regular consumption of strongly acidic vinegar drinks and after tablet preparations.

Worsened reflux

In some people vinegar can increase heartburn, belching, an acidic aftertaste, or upper abdominal pain. It is therefore not a universal home remedy for reflux. Sensitivity is individual, and people with reflux disease often tolerate acidic products less well.

Nausea and fullness

Slower gastric emptying can increase fullness after a meal, but it can also cause nausea, discomfort and bloating.

Possible electrolyte disturbances

Very large amounts of vinegar used over a long time could theoretically affect potassium levels. Cases of hypokalaemia linked to excessive intake have been described. Such situations are rare, but they show that a "natural" product is not always free of risk.

Does everyone need to limit post-meal glucose spikes?

Not every rise in glucose after eating is abnormal. After consuming carbohydrates, blood glucose physiologically rises and then falls thanks to the action of insulin.

In a healthy person, the goal is not to achieve a perfectly flat glucose line. The body is adapted to changes in glucose concentration after a meal.

It is worth paying more attention to:

  • fasting glucose,
  • HbA1c,
  • the results of an oral glucose tolerance test, if indicated,
  • the repeatability of abnormal values,
  • clinical symptoms,
  • the overall quality of the diet,
  • the risk of developing diabetes.

A single high reading from a sensor after an unusual meal does not automatically mean insulin resistance or diabetes.

Apple cider vinegar: hit or myth?

The most honest answer is: a bit of a hit, but above all a heavily exaggerated add-on.

Vinegar can:

  • slightly blunt the post-meal rise in glucose,
  • improve some glycaemic parameters in certain people,
  • serve as a low-calorie ingredient in sauces and marinades,
  • make vegetables and salads more appealing.

There is, however, no convincing evidence that it:

  • causes large and lasting weight loss,
  • cures insulin resistance on its own,
  • burns fat,
  • detoxifies the body,
  • replaces physical activity, diet or pharmacotherapy,
  • neutralizes the effects of a large amount of sugar in a meal.

The biggest mistake is not adding vinegar to a salad. It is attributing to it the properties of a medicine and expecting it to solve a problem that requires comprehensive management.

Summary

Apple cider vinegar is not a miracle remedy for glucose or weight loss. The acetic acid it contains may slightly slow the digestion of carbohydrates and blunt the post-meal rise in glucose. This effect, however, is relatively small and depends on the type of meal, the dose and the individual response of the body.

The evidence on the effect of vinegar on body weight remains limited. The small changes seen in some studies do not allow it to be considered an effective method of treating overweight or obesity. The particularly spectacular results of a popular 2024 study turned out to be unreliable, and the publication was officially retracted.

Vinegar is best treated like a seasoning. It can be part of a healthy diet, especially when it replaces more calorie-dense sauces, but it should not be its foundation. It is also worth remembering that a single product rarely changes the metabolic picture – what matters more is the whole diet in insulin resistance and lifestyle.

If it is well tolerated, you can add it to meals. It is not worth drinking undiluted "shots", exposing your enamel and digestive tract, or changing your diabetes treatment on that basis.

Frequently asked questions

Does apple cider vinegar lower blood sugar after eating?

It may slightly blunt the post-meal rise in glucose, especially after a meal containing starch or other carbohydrates. The effect does not occur in everyone, though, and it does not mean vinegar neutralizes the sugar in your food.

How much apple cider vinegar should you take before a meal?

There is no need to drink vinegar. It is safer to use it as an ingredient in a sauce, marinade or salad. Studies typically used around 10–30 ml per day, but no single optimal dose has been established for everyone. More does not mean better.

Can you drink apple cider vinegar on an empty stomach?

You can, but it brings no proven additional benefit. In sensitive people it may cause nausea, heartburn, burning or stomach pain. Drinking vinegar on an empty stomach does not "switch on" fat burning.

Does apple cider vinegar help you lose belly fat?

No product causes localized burning of belly fat. Vinegar may at most have a small effect on appetite or body weight, but it does not selectively reduce fat tissue from one area.

Does apple cider vinegar cure insulin resistance?

No. It may affect post-meal glucose, but it does not remove the causes of insulin resistance. The foundations of management remain diet, physical activity, sleep, reducing excess body weight and treating coexisting conditions.

Can you take vinegar if you have diabetes?

A small amount of vinegar as a food ingredient is usually acceptable, as long as it is well tolerated. People taking insulin or medications that can cause hypoglycaemia should discuss regular vinegar use with a doctor. Vinegar does not replace pharmacotherapy.

Can apple cider vinegar damage your teeth?

Yes. Regular contact with acidic liquid can cause enamel erosion. The risk is particularly high when drinking undiluted vinegar, sipping it over a long time, or holding it in the mouth.

Does drinking through a straw protect enamel?

It may limit the direct contact of the liquid with part of the teeth, but it does not eliminate the risk. The vinegar still comes into contact with the mouth and can irritate the throat and oesophagus. The best solution is to use it as a meal ingredient.

Are apple cider vinegar tablets safer?

Not necessarily. Preparations can differ in acid content, composition and quality. Irritation and even oesophageal injury after tablet products have been described. A supplement is not necessarily safer than culinary vinegar.

Does apple cider vinegar break a fast?

A small amount of pure vinegar provides very little energy, so it usually has no meaningful effect on your calorie balance. It can, however, irritate the stomach, especially when fasting. If you are fasting before a diagnostic test, follow the laboratory's instructions and do not drink vinegar.

Does vinegar lower the glycaemic index of a meal?

It may reduce the post-meal glycaemic response, but it does not change the actual amount of carbohydrates and energy in the meal. It should not be a way to "fix" every dish.

Do you have to brush your teeth right after vinegar?

No. Brushing immediately after contact with an acidic product can increase the mechanical wear of temporarily softened enamel. It is better to rinse your mouth with water and wait before brushing.
